Eligibility requires that a patient must:

• Be a Medicare beneficiary
• Be willing to participate in the demonstration (participation is voluntary and refusal does not affect Medicare benefits or Home Health Care benefits)
• Meet Home Health eligibility criteria
• Have a need for service that can be safely rendered in a group setting
• Have a need for services that are feasible to be rendered at a medical day care facility and/or in the patient's home
• Have a physician's order
