Neighborly Care Network's Nutrition Services program includes:

With Nutrition Risk Reduction service, Neighborly's licensed dietitians conduct a Nutritional Assessment and Evaluation of clients identified as being at high risk of malnutrition. A dietitian works with the client to develop and implement a Care Plan. The plan includes follow-up visits to the senior by the dietitian to ensure there is positive progression in the senior's nutritional recovery.

Fees and Payment Options
Nutrition Risk Reduction Counseling:  $50.00 per hour
(provided by a licensed dietitian)

Neighborly also offers one-on-one Nutrition Counseling by licensed dietitians. Often, seniors at risk of malnourishment just need to be taught the basics of maintaining a healthy diet. Once aware of the healthiest foods for their particular situations, seniors can independently maintain good nutritional health.

Nutrition Services funding sources include:

  • Older Americans Act
  • Medicaid Waiver
  • Community Care for the Elderly
  • United Way
  • County and Municipal Grants

Private donations and support are also a big help.

Call Neighborly's Nutrition office at (727) 756-1001 to discuss nutrition services and financial assistance possibilities.
