It's about choice.
The Neighborly Care Network choice care plan offers home- and community-based services, both acute and long-term, as an alternative to nursing home care. Qualified individuals receive, at absolutely NO cost, a wide range of services, such as Meals on Wheels, assistance with daily tasks, health care, intensive care management, and pharmacy services, so they can live independently at home.
The choice Program is sponsored by Neighborly Care Network and the State of Florida, Department of Elder Affairs (DOEA), to provide its members with long-term care.
Following are the eligibility requirements for individuals who are interested in becoming members of the Neighborly Care Network choice care plan:

age 65 or older, with both Medicare A&B, and Medicaid eligible

live in Brevard, Broward, Citrus, Dade, Hernando, Hillsborough, Indian River, Manatee, Martin, Orange, Osceola, Palm Beach, Pasco, Pinellas, Polk, Sarasota, Seminole, or St. Lucie county

meet financial requirements determined by the Department of Children and Families

meet clinical criteria for being at risk of nursing home placement
