Help for Participants and Their Caregivers
The professional staff at Neighborly Care Network Adult Day Care Centers assess each participant on an individual basis to develop a comprehensive care plan to address needs in the following areas: physical, emotional, nutritional, and social.
An individual care plan allows the participant to receive the maximum benefit from our therapeutic activity programs, as well as medical and nutritional monitoring. Some examples of the chronic conditions included in our care are:
- Stroke
- Diabetes
- Heart Disease
| - Alzheimer's or other Dementia illness
- Parkinson's Disease
- Depression

Instead of being placed in an Assisted Living Facility or nursing home, a participant who wants to continue living at home and "age in place" may do so by attending Adult Day Care in the day. This allows the working caregiver to remain employed or the one offering 24-hour care to have a much needed respite from the demands of constant care.
Benefits to the participant include:
- Help frail elders continue functioning at a higher level for longer periods of time
- Reduce acute health episodes through a daily regimen of good nutrition, medication monitoring and management, socialization, stimulation, and exercise.
